Application process
Written application required.
Please bring the items listed below to the elementary school located in your attendance zone.
1. An original certified state Birth Certificate or Birth Certificate Affidavit.
2. A certificate of physical examination from a phsician or health department.
3. An immunization certificate that meets all necessary requirements.
4. Proof of residencey, such as, current gas, water, or electric bill.
