Provide "wrap-around" services, focusing on the individual's receiving assistance and developing unique service plans to fit their circumstances and needs. Participants are assigned Care Managers to provide continual, confidential assessments and follow-up. Participants receive in-home assessments to identify their needs and be directly connected to agency programs, including those providing nutrition, transportation, housing, health and wellness, and care services. Advocacy, informational resources, and referrals to other community service organizations are also available.

The goal of case management is to optimize client functioning by providing services to individuals with multiple and complex needs. Case management helps individuals make changes to improve their life situation.

Partners with school systems and other agencies to help students with disabilities in transition from middle school to high school and post graduate opportunities. Helps people transition from nursing facilities and other institutions back into the community.

VPAS' Case Management service offers an assessment of needs, development of an individualized plan of care, referral and linkage to community services, and monitoring and satisfaction of services implemented. A personal home visit is conducted to determine the level of service required.

Provides community-based case management for individuals affected by brain injury. Case management includes identifying needs and developing a plan of action to meet those needs. Help individuals and their caregivers find their way through the rehabilitation process which can last for years following an injury. After determining what an individual needs and would like to accomplish, we identify and coordinate services to help them reach their goals. Monitor progress toward those goals and build partnerships and teams to create a lasting network for continued support. Primary function is to help individuals affected by brain injury return to their lives - to their homes, careers, school and communities - to the greatest extent possible.

Care Coordination connects older individuals with a variety of services and resources that are needed to stay well and independent.

Provides women facing unplanned pregnancies with resources, referrals, adoption services and information, allowing them to make good decisions regarding the health and well-being of both themselves and their unborn child. Offers answers to questions and supports in the pregnancy decision making process. Locates available resources in the community that may be able to assist with medical care and related expenses. Works with both parents, either separately or together, in planning for your baby's needs. Services include: *Free counseling for pregnant women; *Adoption planning; *Referral services; *Follow up and support; *Baby items.

Partnered with Shenandoah Valley Social Services to provide vocational and general case management services for TANF/VIEW participants. Services provided:

Provide an array of outpatient services for individuals age 18+ with mental illness, substance abuse disorders and intellectual disabilities. Services include psychiatric treatment, medication management, case management, in-home services, psychosocial clubhouse, and peer support. Assists low-income families with children ages and 6 and under and pregnant women with access to medical and dental services and acts as a link to other human service organizations/providers in the NRV. Offers comprehensive case management in the home where children receive consistent and comprehensive care in a familiar setting. CHIP services, provided by a nurse and a home visitor team, include helping families make and keep appointments and follow physician recommendations, educational home visits, parenting education, helping families set and reach personal and family goals, transportation to medical and dental appointments, medical case management, and referrals to other community-based services to help meet the multifaceted needs of families. All services emphasize prevention and early intervention. This is not the state CHIP insurance, but will help families access Medicaid or FAMIS (state CHIP insurance). Not a substance abuse/ mental health provider but can connect families to those resources. Transports only if no other means available.

Provides services to help prevent and treat Intellectual Disabilities and to enhance the quality of life of individuals who experience these conditions in the Cities of Buena Vista and Lexington and the Counties of Rockbridge and Bath, Virginia by providing individualized treatments, establishing and expanding programs and support systems, and promoting integration into the community. Offers group homes and adult day activities programs to enhance the quality of life for adults with intellectual disabilities by providing comprehensive support services.

Needs assessment, case management, and referral to a network of providers to help youth exhibiting negative behaviors. TOP (Teamwork ~ Options ~ Pathways to Success) is an interest and needs based prevention program designed to connect youth and their families with services that can refocus youth in the right direction by building on their strengths and providing them with a positive support system.

One on one counseling and mentoring program designed to help people get out of poverty.

The provision of home and community-based services for adults and adults with disabilities to achieve self-sufficiency and to prevent unnecessary institutionalization.

Baby Care: Special assistance to access community services for Medicaid eligible infants aged two years and under that are at risk for poor health. Promotes healthy outcomes for at risk pregnant women and infants through home visiting and education which is provided by registered nurses.

Provides case management services through grade 5 in collaboration with Albemarle County Schools. Family Support provides prevention and case management to K-8 in 11 elementary and 3 middle schools.

Provides free access to HIV and Hepatitis C prevention information/education, HIV Pre-Exposure Prophylaxis (PrEP) and non-occupational Post-Exposure Prophylaxis (nPEP) counseling. Service Navigation for linkage to STI testing, PrEP and nPEP navigation, basic needs (i.e., food, housing, etc.), insurance enrollment, etc., available to anyone regardless of HIV or HCV status.

Home-based care provides personal aide and hygiene care, light housekeeping, supervision and other supportive services to elderly and disabled adults to the extent that funds are available, so that they may remain in the community and out of institutional placement. Adults must meet all eligibility criteria to qualify and there may be a waiting list for service.

The DARS Community Rehabilitation Case Management Services (CRCMS) Program assists people with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources. CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver. Helps young clients and their families to access available resources. Case managers provide assessments, long-term treatment planning, coordination of services, client advocacy, support outreach, and crisis prevention. Limited client transportation is available.

Bilingual case managers will conduct a comprehensive assessment of Latinos and others to obtain health care services, cultural, and social needs and develop a patient-centered care plan with mutually agreed upon goals.

Case Management Services for adults collaborates with individuals to access the supports and services they need to meet their basic needs and to provide guidance on their journey as they work towards meeting their personal goals.

Project LINK offers services to women and children whose lives have been negatively affected by substance use. This may include the woman’s own substance use, her partner’s use, or use by a family member. Priority services are given to women who are currently pregnant, parenting, or seeking to regain custody. The program provides comprehensive, individualized, family-focused, gender specific case management services.

JFS offers each client an individualized Care Management plan based on a comprehensive assessment. Care Management staff will work and discuss services you would like to receive to enhance your quality of life. Once a plan is set in motion, we review it regularly to ensure your needs and expectations are being met.

Free ongoing support for families of children/youth with special needs. Help families to coordinate care and to connect with the medical and community-based services specific to each child’s individual needs, including special education. We offer information and resources to anyone seeking information about children with disabilities (enrollment not required).
